Self-Managed U.S. Application Checklist – Neuroscience q The original and two photocopies of the Graduate College’s Application for Graduate Admission. (If you have applied electronically, you must include 2 printed copies of the online application.) Multiple applications: If you are applying to more than one graduate program, you must send a self-managed application packet for EACH program. However, additional fees are NOT required. q Nonrefundable application fee of $30. Make check or money order payable to Iowa State University. (If you applied electronically to the Graduate College and paid it by credit card, please check below.) q Fee paid by credit card. q Two official academic transcripts from each previous institution attended, in sealed envelopes. (Some colleges and universities will not mail official transcripts to students. In this instance, please send us your STUDENT COPY of the transcripts with your self-managed application, and have the institutions send the official transcripts directly to the Office of Admissions.) q Three letters of recommendation. Please have letters placed in a sealed letterhead envelope that has the recommended’s signature across the seal. q Examination Results: You must arrange to have your official examination results sent directly to the university from the testing agency. If you have received your results, please include TWO photocopies of each examination results in your self-managed application packet. q Graduate Records Examination (GRE) and/or appropriate Subject Test q Test of English as a Foreign Language (for nonnative English speakers only.) q The Neuroscience Program requires additional items to be included in your self-managed application packet. The following items should also be included in your application. Please check below those that you have included in your self-managed application packet. q Neuroscience Program Application q Statement of Purpose, Goals, and Research Interests q Resume or Curriculum Vitae (Please include a description of previous honors, scholarships, research activities, publications, and outstanding extracurricular work.)
